Pharmacological action of

Cocarboxylase catalyzes the oxidative decarboxylation of alpha-keto acids (pyruvic, alpha-ketoglutaric, etc.).

Indications

Metabolic acidosis, hyperglycemic coma and acidosis in diabetes mellitus, liver and kidney failure, respiratory acidosis in chronic, cardiopulmonary insufficiency, pulmonary insufficiency, circulatory failure Ischemic heart disease, myocardial infarction and post-infarction cardiosclerosis (as part of complex therapy), acute alcohol poisoning and chronic alcoholism poisoning with digitalis drugs, barbiturates, intoxication in infectious diseases: diphtheria, scarlet fever, typhoid and paratyphoid (in complex therapy), peripheral neuritis.

Use during pregnancy and lactation

Dosage and administration of

Cocarboxylase is administered s / c, v / m. The dose is set individually, depending on the nature of the disease and the severity of the patient's condition. Adults: 50-100 mg / day, once, if necessary (diabetic coma) - again after 1-2 hours later they switch to a maintenance dose of 50 mg / day for circulatory failure 50 mg is administered 2 hours before the use of digitalis 2- 3 times a day. Children per day (in 1-2 doses): up to 3 months - 25 mg, from 4 months to 7 years - 25-50 mg, from 8 to 18 years old - 50-100 mg.

Drug Interaction

Due to the presence of pyridoxine (vitamin B6), co-administration with levodopa is contraindicated due to the accelerated metabolism of levodopa with the participation of the pyridoxine-dependent enzyme. To prevent this interaction, it is possible to use an inhibitor of dopa-decarboxylase - carbidopa.

Due to the presence of folic acid in the preparation, caution should be exercised when co-administered with antiepileptic agents containing phenobarbital, phenytoin or primidone. Clinical monitoring is required and, if possible, control of the content of antiepileptic drugs in the blood plasma, correction of the dosage regimen of the antiepileptic drug during and after discontinuation of folic acid.

Compatibility with other infusion solutions should be checked, especially if Cernevit is added to binary parenteral mixtures containing glucose, electrolytes and amino acid solutions, or to mixtures containing glucose, electrolytes, amino acid solutions.

Overdose

Overdose is predominantly caused by excess doses of vitamin A.

Symptoms: with an acute overdose of vitamin A (doses exceeding 150,000 ME) - gastrointestinal disorders, headache, increase in intracranial pressure, edema of the optic nerve, mental disorders, excitability, sometimes convulsions, delayed generalized desquamation of the epithelium in chronic overdose of vitamin A - increase in intracranial pressure, cortical hyperostosis and premature infections fingers of the upper and lower extremities. X-ray examination of the elbow, fibula, clavicle and costal bones shows diaphyseal periosteal thickening.

Treatment: discontinue use of the drug, reduce the use of calcium, increase diuresis and conduct adequate rehydration of the body.
