Pharmacological action

ETAPERAZINE - an antipsychotic agent (antipsychotic), piperazine derivative of phenothiazine. It is believed that the antipsychotic effect of phenothiazines is due to blockade of postsynaptic dopamine receptors in the mesolimbic structures of the brain. Perphenazine has a strong antiemetic effect, the central mechanism of which is associated with inhibition or blockade of dopamine D2 receptors in the chemoreceptor trigger zone of the cerebellum, and peripheral blockade of the vagus nerve block in the gastrointestinal tract. It has alpha-blocking activity. Anticholinergic activity and sedation may occur from mild to moderate intensity, hypotensive effect is weak. It has a pronounced extrapyramidal effect. The antiemetic effect can be enhanced by anticholinergic and sedative properties. It has a muscle-relaxing effect.

Indications

Treatment for psychotic disorders, especially with hyperactivity and agitation, schizophrenia neurosis, accompanied by fear, tension. Treatment of nausea and vomiting of various etiologies. Itchy skin.

Contraindications

Cirrhosis, hepatitis, hemolytic jaundice, nephritis, hematopoiesis, myxedema, progressive systemic diseases of the brain and spinal cord, decompensated heart disease, thromboembolic diseases, late stages of bronchiectasis, pregnancy, lactation, increased sensitivity.

Use during pregnancy and lactation

Perphenazine is contraindicated in pregnancy and lactation.

Dosage and administration of

For adults and children over 12 years of age, when taken orally, the daily dose is 4-80 mg. In the chronic course of the disease and in resistant cases, the daily dose can be increased to 150-400 mg. The frequency of administration and the duration of the course of treatment are set individually.

Side effects of

From the side of the central nervous system: drowsiness, akathisia, blurred vision, dystonic extrapyramidal reactions, parkinsonian extrapyramidal reactions.

From the liver: rarely - cholestatic jaundice.

From the hematopoietic system: rarely - agranulocytosis.

From the side of metabolism: rarely - heat stroke, melanosis.

Allergic reactions: rarely - a skin rash accompanied by contact dermatitis.

Dermatological reactions: rarely - photosensitivity.

Effects due to anticholinergic action: dry mouth, accommodation disturbances, constipation, difficulty urinating are possible.

Drug Interaction

When combined with CNS depressant drugs, ethanol, ethanol-containing drugs may increase CNS depression and respiratory function.

At simultaneous use with anticonvulsants it is possible to lower the threshold of convulsive readiness with the means for the treatment of hyperthyroidism - the risk of developing agranulocytosis is increased.

When used with drugs that cause extrapyramidal reactions, the frequency and severity of extrapyramidal disorders may increase.

When used concurrently with drugs that cause arterial hypotension, pronounced orthostatic hypotension is possible.

When used with agents with anticholinergic effects, their anticholinergic effects may be enhanced, while the antipsychotic effect of the neuroleptic may be diminished.

When used with tricyclic antidepressants, maprotiline, MAO inhibitors, the risk of developing ZNS increases.

With the simultaneous use of antacids, antiparkinsonian agents, lithium salts, the absorption of phenothiazines is impaired.

At simultaneous application it is possible to reduce the effect of amphetamines, levodopa, clonidine, guanetidine, epinephrine.

When used with fluoxetine, extrapyramidal symptoms and dystonia may develop.

When used concomitantly, the vasoconstrictive effect of ephedrine may be impaired.
