Pharmacological action
Pharmgroup: glucocorticosteroid for topical use.
Pharmaceuticals: Lokoid is a synthetic non-halogenated glucocorticosteroid for external use. The esterification of the hydrocortisone molecule with the remainder of butyric acid (butyrate) allowed a radical increase in the activity of the drug compared to native hydrocortisone. Lokoid has a rapidly onset anti-inflammatory, decongestant, antipruritic effect.
The use of Lokoid in recommended doses does not cause suppression of the hypothalamic-pituitary-adrenal system. Although the use of large doses of the drug for a long time, especially when using occlusive dressings, can lead to an increase in the content of cortisol in the blood plasma, this is usually not accompanied by a decrease in the reactivity of the pituitary-adrenal system, and drug withdrawal leads to a quick normalization of cortisol production.
Pharmacokinetics: After application, the active substance accumulates in the epidermis, mainly in the granular layer, systemic absorption is negligible. A small amount of hydrocortisone 17-butyrate is absorbed into the systemic circulation unchanged.
Most hydrocortisone 17-butyrate is metabolized to hydrocortisone and other metabolites directly in the epidermis and subsequently in the liver.
Metabolites and a small portion of unchanged hydrocortisone 17-butyrate are excreted by the kidneys and through the intestines.

Contraindications

In childhood, suppression of adrenal cortex function may develop faster. In addition, a decrease in growth hormone excretion may be observed. When using the drug for a long time, it is necessary to monitor body weight, height, plasma cortisol level. In a study in children who received 30-60 g of Lokoid per week for 4 weeks in comparison with 1% hydrocortisone ointment, not one child showed dysfunction of the adrenal cortex, with differences in hydrocortisone 17-butyrate in this indicator from usual hydrocortisone was not observed.
Lokoid should not be applied to the periorbital area due to the risk of developing glaucoma. With a systematic exposure to the conjunctiva, there is a risk of an increase in intraocular pressure.
When localizing the process on the scalp and exudative inflammation, it is preferable to use Lokoid in the form of a Crelo.
In cases of resistant disease, for example, when localizing dense psoriatic plaques on the elbow areas, knees, severe lichenification, dryness, it is better to give preference to Lokoid in the form of ointment or Lipocream, if necessary, under occlusive dressings.
