Composition of

active substance: trifluoperazine hydrochloride 5 mg

excipients: refined sugar, potato starch, aerosil, calcium stearate, gelatin, basic magnesium carbonate, indigo carmine, polyvinylpyrrolidone, wax, titanium dioxide, talc.

Side effects

From the nervous system: drowsiness, dizziness, insomnia, phenomena of mental indifference (with prolonged use), delayed reactions to external irritations. The use of triftazine is often accompanied by extrapyramidal disorders (dyskinesias, akinetorigid phenomena, akathisia, hyperkinesis, tremors, autonomic disorders), in isolated cases, convulsions. Antiparkinsonian drugs such as tropacin, trihexyphenidyl (cyclodol) and others are used as correctors. Dyskinesias (paroxysmal cramps in the muscles of the neck, tongue, bottom of the mouth, oculogyric crises) are stopped with sodium caffeine benzoate (2 ml of a 20% solution subcutaneously) and atropine (1 ml of 0.1% solution subcutaneously).

With prolonged use, the development of tardive dyskinesia, less commonly, malignant antipsychotic syndrome, is possible.

From the senses: paresis of accommodation, with prolonged use - retinopathy, clouding of the lens and cornea.

From the genitourinary system: urinary retention, decreased potency, frigidity, decreased libido, ejaculation disorders, priapism, oliguria.

From the endocrine system: hypo- or hyperglycemia, glucosuria, amenorrhea, hyperprolactinemia, dysmenorrhea, galactorrhea, swelling or pain in the mammary glands, gynecomastia, weight gain.

From the digestive system: dry mouth, decreased appetite, constipation, bulimia, nausea, vomiting, diarrhea, gastralgia, cholestatic jaundice.

On the part of the sensory organs: visual impairment - paresis of accommodation (at the beginning of treatment), retinopathy, clouding of the lens and cornea.

Laboratory indicators: thrombocytopenia, lympho- and leukopenia, increased blood coagulation, anemia, agranulocytosis (more often at 4-10 weeks of treatment), pancytopenia, eosinophilia - less often than other phenothiazines, false-positive pregnancy tests.

From the cardiovascular system: tachycardia, decreased blood pressure (including orthostatic hypotension) especially in elderly patients and people suffering from alcoholism, heart rhythm disturbances, lengthening of the QT interval, decrease or inversion of the T wave. Allergic reactions: skin rash, urticaria, angioedema (less often than other phenothiazines).

Other: pigmentation of the skin and conjunctiva, discoloration of the sclera and cornea, decreased tolerance to high temperatures (up to the development of heat stroke), melanosis.

Local reactions: intramuscular administration may cause infiltrates, and if liquid forms get on the skin - contact dermatitis.

Drug interaction

With the simultaneous use of triftazine with other drugs, maprotilin or monoamine oxidase inhibitors (MAOs) - increased risk of developing antipsychotic malignant syndrome with anticonvulsants - possibly lowering the seizure threshold with drugs for treating hyperthyroidism - increasing the risk of agranulocytosis with other drugs causing extrapyramidal reactions - possibly increasing the frequency and severity of extrapyramidal disorders with hypritic srdlp - severe orthostatic hypotension with ephedrine is possible - vasoconstriction is weakened effect of ephedrine. In the treatment with triftazine, administration of epinephrine (adrenaline) should be avoided, as the effect of epinephrine can be distorted, which can lead to a drop in blood pressure. The antiparkinsonian effect of levodopa is reduced due to blocking of dopamine receptors. Triftazine can suppress the effects of amphetamines, clonidine, guanethidine. Triftazin enhances the anticholinergic effects of other drugs, while the antipsychotic effect of antipsychotics may decrease.

With the simultaneous use of triftazine with prochlorperazine, prolonged loss of consciousness may occur.

Combination with lithium preparations increases the risk of extrapyramidal complications. Triftazin may mask some manifestations of ototoxicity (tinnitus, dizziness), drugs that have an ototoxic effect (e.g. antibiotics). Other hepatotoxic drugs increase the risk of hepatotoxicity. Antacids containing Al3 + and Mg2 + reduce the absorption of triftazine.

Overdose

Symptoms: areflexia or hyperreflexia, fuzzy visual perception, cardiotoxicity (arrhythmia, heart failure, decrease in blood pressure, shock, tachycardia, change in QRS complex, fibrillation, fibrillation convulsions, disorientation, drowsiness, stupor or coma mydriasis, dry mouth, hyperpyrexia or hypothermia, muscle rigidity, vomiting, pulmonary edema or respiratory depression.

Monitoring of cardiovascular function for at least 5 days, CNS function, breathing, body temperature measurement, psychiatrist consultation. Dialysis is ineffective.
